This is the Identification step in which we actively identify and reach out to street-involved individuals through outreach programs, feeding programs, community partnerships, and engagement with local organisations. This involves building trust, forming relationships, and understanding the unique circumstances of each individual as we seek to identify those who are ready and willing to leave the streets and reintergrate into society. Building trust and rapport is crucial during this phase to encourage street individuals to consider rehabilitation.
We also seek to establish rapport by demonstrating empathy, respect, and a genuine interest in their well-being. Building a sense of belonging and trust is crucial in encouraging them to consider the rehabilitation process.
Assessment and Needs Identification: Once individuals express interest in rehabilitation, we conduct thorough assessments to understand their unique circumstances, challenges, and needs. This helps us tailor our rehabilitation plans to address specific issues such as trauma, substance abuse, mental health concerns, and skill development. This evaluation guides our approach to designing a tailored rehabilitation and reintegration plan. We also help them get registered for the National Hospital Insurance Fund, making it possible and easy to get government services.
This involves Counseling and Therapy: Mental health and emotional well-being play a crucial role in the rehabilitation process. Street individuals often face various psychological challenges due to their experiences. We provide counselling and therapy sessions to address trauma, build coping mechanisms, and develop a positive mindset for their reintegration journey.
Based on the assessment results, we develop individualized rehabilitation plans for each street individual. These plans encompass a range of services, including counselling, medical care, and life skills development. We involve the individuals in setting their own goals for rehabilitation and reintegration. This empowers them and increases their commitment to the process. During rehabilitation, we provide safe and supportive transitional housing where individuals can focus on their healing and skill-building. This environment fosters a sense of belonging and encourages positive behaviour changes.
Skill Development and Education: Equipping street individuals with practical skills and education is essential for their successful reintegration. We offer vocational training programs, skill-building workshops and life skills that align with their interests and strengths and equip them with practical skills to enhance their employability and self-sufficiency.
We also seek to offer Education for those interested; educational opportunities are provided, which can include basic literacy skills or more advanced education based on their preferences and abilities.
During this phase, we also facilitate interactions with peers who have undergone similar experiences, helping street individuals develop a sense of camaraderie and mutual support. Engaging in social activities and community events further promotes their integration.
This stage lasts for a maximum of 9 months.
After completing their Vocational training, we help reintegrate the boys back into the society.
This step involves the following:
1. Reintegration : As individuals near the end of their rehabilitation journey, we focus on their reintegration into society. Our approach involves gradually reintegrating them into the community. This includes involving them in community activities, encouraging positive social interactions, and helping them build supportive networks. We also help them secure stable housing, employment, and ongoing support.
2. Family and Community Reconnection: Whenever possible, we work towards reconnecting street individuals with their families or communities. This process involves mediation, communication, and addressing underlying issues that may have led to their initial separation. Rebuilding familial or community connections provides a support system during the reintegration phase.
3. Progress Monitoring and Support: Regular progress assessments help us track the individuals’ development and make necessary adjustments to their rehabilitation plans. Continued counselling and support are provided to ensure they stay on the path of positive change.
4. Follow-Up and Aftercare: Even after successful reintegration, we continue to provide support through follow-up programs, counselling, and access to resources to ensure their sustained well-being and prevent relapse. We also maintain follow-up programs to ensure their continued success and address any challenges that may arise.
Ultimately, our goal is to offer a holistic and individualised approach that supports street-involved individuals in leaving behind their challenging circumstances and transitioning towards a more stable, fulfilling life within their communities.
We regularly assess and adapt our methods based on outcomes and feedback. This ensures that our approach remains effective and responsive to the evolving needs of street-involved individuals.
